Overview: "Standard forms or interview checklists, when used by interviewers, will make it possible to establish a factual basis on which to evaluate institution-wide terminations, and a clear basis also for interpreting the employees' responses and attitudes. These forms should be filled out after the interview. There should be a series of questions with spaces for short answers. The questions should relate to the employee's new job and reasons for taking it; adequacy of training for the job he/she is leaving; challenge presented and satisfaction obtained while employed; relationship with supervisor; and the employee's assessment of current personnel practices. The interviewer should enlist the employee's cooperation at the outset by encouraging the employee to offer suggestions for improving the work situation. The person conducting the interview should avoid taking a value stand on the truth (or untruth) of the statements made by the employee."
