Overview: To bill the Virginia Medicaid Program for medical supplies and equipment provided to Medicaid eligible recipients, the provider must have authorization for certain supplies or equipment provided. Providers who bill Medicaid for medical supplies, equipment, and appliances must obtain authorization from DMAS when required. This document provides the requirements and instructions on the medical supplies and equipment billing procedure.
