Overview: In progressive neurological disorders, such as multiple sclerosis (MS), magnetic resonance imaging (MRI) follow-up is used to monitor disease activity and progression and to understand the underlying pathogenic mechanisms. A specific aim in MRI assessment of MS is to differentiate among inflammation, brain parenchymal degeneration, and reparatory processes, such as remyelination or fibrillary astrocytosis. Although MRI appears to be collectively sensitive to these changes, MRI-derived surrogate metrics thus far lack specificity in differentiating between them.
